服務須知

  1. 我們接受及授受中心所提供的各項專業心理服務，皆由經過專業訓練、具備專業資格及受督導之人員提供，以確保一切服務均符合專業標準。我們會盡力與中心配合服務時間與內容，並與提供專業工作的人員保持良好合作，務求達致服務成效。

  2. 我們是自願接受服務，我們個人資料的保密將符合倫理原則與法律保密規定，只有在我的同意下（若當事人涉及性侵害案件，則會交由法律處理人員處理）才會向中心以外的人透露情況。若在評估中出以下情況，中心則有權根據實際情況，並將上述資料交予合適的轉介機構與人士（包括我的家人及警察）：
    a. 有傷害自己或他人生命安全的行為；
    b. 對青少年人構成危害的情節；
    c. 任職教師或公務員，有性騷擾行為；
    d. 進行非法性行為使對象感染性病，影響受害人健康或導致其身分暴露。

  3. 為確保服務及評估品質與成效，會談內容可能會被錄音或錄影，以便提供督導或用作專業訓練資料。
    a. 評估過程或治療過程若需書面報告，將會根據項目與範圍收費。
    b. 書面報告會於評估後一個月內完成。
    c. 如需取消預約，請至少提前的時間於上班工作天通知，否則將收取港幣800元或會談費用之50%，以較高者為準的行政費用。

  4. 每節輔導或治療標準時間為60分鐘，如需加時，將會按比例收費。

  5. 根據來訪者的需求，本本機可提供整體心理評估：
    a. 了解來訪者的療癒需求（約約15分鐘）；
    b. 進行評估（視不同評估種類而定）；
    c. 評估過程性回饋（評估後15分鐘）；
    d. 書面報告會於評估後一個月內完成；若需要多方位完整醫療評估，則需另收行政費。

  6. 我們接受及授受：錄音方式進行，否則中心不會對上述服務進行錄音或錄影。我們需要對以文字形式對話內容作記錄及作個案會議記錄、作為後續參考。

  7. 我承諾在進行心理服務之前期間，暫不會飲酒和濫用及不必要的藥物。

  8. 如我本人或受約定的治療服務，有任何傳染病風險（例如：結核病、流感、染疹、咳嗽、嘔吐、腹瀉、肌肉酸痛等症狀），我承諾會提前向中心職員說明。

  9. 服務將於當事人方面的不符配合時終止，除中心必須依法協助有關事宜外，如任何疑問，可直接向中心查詢。

  10. 有 “輔導過程中會引發的法律訴訟”，當事戶主在輔導前簽署聲明書：
    如發生爭議及法律訴訟等事宜，中心將不會為當事人出庭作證；本中心保留隨時終止輔導服務的權利，維持專業操作與支持；如違反情節嚴重者將終止其接受服務；本中心一律不會開立病假證明，各戶須以精神科醫師之證明為主。

  11. 如有任何異議，中心有權保留修正條文之權利。

Service Consent

  1. All professional psychological services provided by our center are delivered by trained professionals who are qualified and supervised. To ensure that all services meet professional standards, we will actively cooperate with the center’s arrangements and service schedule and maintain a collaborative relationship with the service provider for optimal results.

  2. We voluntarily receive services. Our personal information will be kept confidential in accordance with ethical and legal regulations. Disclosure of information outside the center will only occur with our consent (if minors are involved in sexual abuse cases, the matter will be handled by legal authorities). However, if any of the following situations arise during assessment, the center has the right to act accordingly and provide the relevant information to appropriate institutions or personnel (including my family or the police):
    a. Behavior that poses a danger to oneself or others;
    b. Situations endangering the well-being of minors;
    c. Sexual harassment by teachers or civil servants;
    d. Engagement in illegal sexual activities resulting in the transmission of STDs and potentially exposing the identity of the victim.

  3. To ensure the quality and effectiveness of services and assessments, session content may be audio or video recorded for supervision and professional training purposes.
    a. If written reports are required during assessment or treatment, they will be charged based on the scope and type of the report.
    b. Written reports will be completed within one month after the assessment.
    c. If you need to cancel an appointment, please inform the center at least one full working day in advance. Failure to do so will result in an administrative charge of HKD 800 or 50% of the consultation fee, whichever is higher.

  4. Each counseling or treatment session is scheduled for 60 minutes. Any overtime will be charged proportionally.

  5. Based on the client’s needs, the center can provide a comprehensive psychological evaluation:
    a. Understand the client’s therapeutic needs (approximately 15 minutes);
    b. Conduct evaluation (depending on the specific evaluation method);
    c. Provide feedback on the evaluation (15 minutes post-assessment);
    d. Written reports will be completed within one month. For a comprehensive medical evaluation by multiple professionals, additional administrative fees will apply.

  6. We accept and agree to recording (audio) of sessions. Otherwise, the center will not record the service session. We also agree that written records of conversations and contents may be kept for internal case meetings and future reference.

  7. We promise not to consume alcohol or misuse unnecessary medications before or during the psychological services.

  8. If I or my agreed therapy participant has any contagious diseases (such as tuberculosis, flu, rash, cough, vomiting, diarrhea, muscle soreness, etc.), I agree to notify the center staff in advance.

  9. Services will be terminated if the participant fails to cooperate. Apart from legal obligations, the center reserves the right to suspend services at any time. Please contact the center if you have any questions.

  10. In the case of legal proceedings arising during the counseling process, the client must sign a declaration beforehand:

  • If legal disputes or litigation arise, the center will not appear in court as a witness.

  • The center reserves the right to terminate services to maintain professional integrity.

  • If violations are serious, the client will be terminated from receiving services.

  • The center does not issue medical leave certificates; clients must obtain them from a licensed psychiatrist.
